For your convenience,you can download, print, and complete the initial paperwork and consent forms before your first appointment. You will review these documents during your first session and go over any questions you may have with your provider. Please also bring your insurance card, as well as your form of payment. Co-payments and fees for services are due at the time of service. Note: the credit card authorization form is required for new patients. All personal, clinical, and financial information is kept strictly confidential and protected in HIPAA compliant, secured electronic medical records.
For adolescents, parent or legal guardian must accompany patient to the first appointment and provide consent. If parents are married, or both parents have joint legal custody, both parents must sign consent forms before patients under 18 can be seen. Please call to discuss your individual circumstance if this is not possible. We can work out obtaining a faxed or scanned email document if one parent is off island.
